What is this?
Use this form to request assistance with the IT technology in your classroom or meeting room.
Use of this form is reserved for non-urgent requests only, if your class or meeting is in-session, please use the In-Progress Meeting and Classroom Support ticket form.
How is this used?
If the IT technology in your classroom or meeting room has been problematic, but it isn't disrupting an in-session class or meeting, use this form to report the problem so we can schedule a time to fix it.
Please remember that ITS hours of operation are 8.00 am - 4.00 pm Monday to Friday, and immediate, in-person assistance is dependent upon available resources.
What information do I need to use this form?
- What is your location?
- Which campus are you at?
- Which room are you in? If the form does not support your room number, please supply it in the description box.
- How can we contact you? If you're at another campus, or we need to support you remotely, how can we connect with you?
- What piece of equipment isn't working?
- What is happening? Please describe your problem in as much detail as you can. Please provide a screen shot of your entire screen, or even a photograph, if you're presented with an error message. If we can see what you see, it will help us solve your problem faster.
- What troubleshooting steps have you tried? Did you check that everything is plugged in? Are all the cables connected? Did you turn it off and on again? Here are some basic things to try.
