What is this?

Use this form to request support for or to report broken, malfunctioning, or stolen computer hardware

What information will I need to submit this ticket?

In order to fix the problem quickly, we'll need to know:

• What kind of equipment you're having trouble with
• Your location - Where is the broken equipment? Which campus are you on? What is your room number?
• When is the workstation accessible for support attempts?
• Detailed information regarding the issue, including photographs or screen shots if possible
• What troubleshooting steps have you taken? Did you check that everything is plugged in? Are all the cables connected? Did you turn it off and on again? Here are some basic things to try.
